Pre-Event Questionnaire
This form should be filled out 6 weeks in advance of your event and returned to the Social Committee. This is a "fillable" form. You can download it to your device, fill it out, save and email the file to the Social Committee at MOSNSocial@gmail.com, or print it out and hand deliver to a Social Committee Co-Chair. It will help the Social Committee with information to provide to the Community, Ticket Sales and Postings.

Table Layouts for Clubhouse 2
The attachment in this section includes 5 table layouts for the Clubhouse 2 Ball Room and Card Room. Please pick the layout that best suits your event. When filling out your Pre-Event Questionnaire, write the letter for your preferred Table Layout (letters A through E) in the corresponding space on the Pre-Event Questionnaire form.

Social Event Expense Sheet
This form should be filled out within 2 weeks after your event and returned along with receipts to the Social Committee. The detail on this sheet will be extremely important to future hosts!
This is a "fillable" form. You can download it to your device, fill it out and email the file along with your receipts (scanned in PDF format) to the Social Committee at MOSNSocial@gmail.com, or print it out and hand deliver along with paper copies of your receipts to a Social Committee Co-Chair.

Social Non-Event Expense Check Request
This form is for miscellaneous Social Committee expenses NOT related to an event. It should be returned along with receipts to the Social Committee. The information on this sheet will help our Treasurer process your reimbursement.
This is a "fillable" form. You can download it to your device, fill it out and email the file along with your receipts (scanned in PDF format) to the Social Committee at MOSNSocial@gmail.com, or print it out and hand deliver along with paper copies of your receipts to a Social Committee Co-Chair.

Please note, a Table Layout attachment was added to the main post so that Hosts can select the best layout for their event.
Also, both the Pre-Event Questionnaire and the Social Event Expense Sheet have been replaced with revised forms.
And, a new form, the Social Non-Event Expense Check Request form has been added (for reimbursement of Social expenses NOT related to an Event
